    CONDITIONS OF APPLICANT RELEASE FORM

  1. Applicant represents and agrees that he/she is in good physical condition and that he/she has no disability, impairment or ailment preventing him/her from engaging in strenuous physical activity or that will be detrimental or inimical to his/her health, safety, comfort or physical condition.
  2. Applicant agrees that PREP Physical, LLC shall not be liable for any damages arising from personal injuries sustained by the Applicant in, or about the premises or as a result of using the facilities and equipment therein.
  3. Applicant assumes full responsibility for any such injuries or damages which may occur to him/her in, or about the premises and does hereby fully and forever release and discharge PREP Physical, LLC, its owners, employees and agents from any and all claims, demands, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the Applicant�s use or intended use of said facilities and equipment thereof.
  4. Applicant acknowledges that PREP Physical, LLC has made no claims as to physical results which can or may be obtained by applicant through the use of PREP Physical, LLC’s program.
  5. PREP Physical, LLC shall not be liable for the disappearance, loss or theft of, or damage to the personal property of Applicant.
  6. Violation of the Conditions of Applicant is subject to termination, at the sole discretion of PREP Physical, LLC. No fees shall be refunded.
